THE PRIMARY PURPOSES OF THE POSITION OF COMMUNITY ASSESSMENT (CHA) MANAGER ARE TO PROVIDE OVERALL MANAGEMENT OF THE N.C.COMMUNITY HEALTH ASSESSMENT SYSTEM, TO ADMINISTER THE CDC GRANT, COMMUNITY ASSESSMENT INTEGRATION PROJECT, AND MANAGE THE STATEWIDE TRAINING FOR PUBLIC HEALTH WORKFORCE FOR COMMUNITY HEALTH ASSESSMENT. NC COMMUNITY HEALTH SYSTEM: CHA MANAGER MONITORS AND DOCUMENTS LOCAL HEALTH DEPARTMENTS (LHD) COMPLIANCE WITH SUBMITTING CHA DOCUMENTS AND STATE OF THE COUNTY HEALTH (SOTCH) REPORTS FOR EACH SUBSEQUENT YEAR. N.C. CHA INTEGRATION PROJECT GRANT: CHA MANAGER IS TO ENSURE THAT (A) GOALS AND OBJECTIVES ARE BEING MET, (B) PROPER DOCUMENTATION AND MATERIALS ARE BEING SUBMITTED TO PROJECT AND BUDGET OFFICER IN A TIMELYMANNER, AND (C) REMAIN IN COMPLIANCE TO THE RULES AND REGULATIONS OF THE GRANT TO CONTINUE TO BE FUNDED. CHA MANAGER MANAGES STATEWIDE TRAINING: (A) TO STRENGTHEN COMMUNITY HEALTH EDUCATION WORKFORCE ON COMMUNITY HEALTH ASSESSMENT PRACTICES AND(B) TO MEET THE STATE-MANDATED REQUIREMENTS AND ACCREDITATION STANDARDS FOR COMMUNITY HEALTH ASSESSMENT (CHA) AND SOTCH REPORT.

CONSIDERABLE KNOWLEDGE, SKILLS & ABILITIES IN: PUBLIC HEALTH KNOWLEDGE OF NORTH CAROLINA AND NATIONAL HEALTH OBJECTIVES (E.G., HEALTHY PEOPLE 2010). SKILLS IN DESIGNING AND IMPLEMENTING FORMATIVE, PROCESS, IMPACT AND OUTCOME EVALUATION OF PUBLIC HEALTH INTERVENTIONS. SKILLS IN COMMUNITY ORGANIZATION AND MOBILIZATION. SKILLS IN PUBLIC HEALTH SOCIAL MARKETING, COMMUNICATION, AND MEDIA RELATIONS. KNOWLEDGE OF CURRENT BEST PRACTICES IN HEALTH EDUCATION, COMMUNITY HEALTH ASSESSMENT AND ACCREDITATION STANDARDS. SKILLED IN IDENTIFICATION, ANALYSIS AND REPORTING OF SECONDARY DATA. SKILLED IN COLLECTION, ANALYSIS, AND REPORTING OF PRIMARY DATA. UPDATE AND MAINTAIN COMMUNITY HEALTH ASSESSMENT COUNTY-TO-STATE DATA SYSTEM. EXPERIENCE IN PLANNING, IMPLEMENTING AND EVALUATING TRAINING AND FOLLOW-UP IN ALL ASPECTS OF PARTNERSHIP DEVELOPMENT, COMMUNITY HEALTH EDUCATION AND COMMUNITY HEALTH ASSESSMENT AND IN DEVELOPING TRAINING MANUALS/MATERIALS.
