Local Access to Coordinated Healthcare (LATCH) and the Durham Community Health Network (DCHN) are health care management programs that strive to improve health outcomes and access to healthcare for uninsured and Medicaid patients living in Durham County. The LATCH & DCHN Programs are currently seeking a full-time, bilingual (Spanish/English) Health Education Specialist to coordinate healthcare services and other community resources for individuals and families. The Health Education Specialist will also assist both programs in developing a patient education portfolio of materials, resources and activities that can be used by the Care Management Teams to address various health issues with patients. Candidates should possess the following characteristics, knowledge and/or skills: • Fluency in English and Spanish, both written and verbal; • Experience conducting in-home care management and patient education in underserved communities; • Familiarity with Durham County’s health care safety net, social services programs and other community resources; • At ease working with individuals from diverse backgrounds and cultural belief systems; • Motivated and self-directed with a capacity to independently manage a patient caseload; • Ability to work effectively with medical professionals, community agencies, and clients in various settings; • Team-oriented and able to maintain positive working relationships with co-workers; • Excellent organizational skills, flexibility, and ability to prioritize work demands; • Previous experience in public health, community health, health education, or social work. • A valid NC driver’s license and a clean driving record. Position requires a bachelors degree in health education, public health, social work, or related health field. Candidates with at least two years of experience working in community-based health programs preferred.
